Work Schedule:
Two Shifts Available:
- Graveyard (12 am - 8 am Weekdays; 12 am - 12 pm Weekends)
- Swing Shift (4 pm - 12 am Weekdays, 12 pm - 12 am Weekends)

Job Description:
- Employee will work with participant on Daily Living Skills (Cooking, Cleaning, Time management, money management, hygiene, social skills, etc.)
- Employee will help/assist client:
- Cooking food according to meal plan
- Keep personal and shared living spaces clean, healthy, and safe
- Manage appointments and social events and get to them in a timely manner
- Follow monthly and weekly budget
- Take showers, shave, put on deodorant, teeth brushing, and all other personal hygiene
- Create and maintain relationships
- Employee will transport clients to all appointments (When Scheduled) and social outings (at least once per shift) in their own vehicle (as long as you are approved by office to drive).
- Employee will provide safe and continuous support in the home or community by maintaining arm’s length/line of sight/awake and alert posture and provisions throughout any shift
- Employee will provide recreation and leisure activities, teach functional communication, implement approved behavior plans, and achieve increased independence, productivity, and inclusion in the community
- Employee will complete data on a daily basis in line with the expectations of the team and ensure proper completion of Client times, ADL's, goals/support strategies, and MARS
- Must be at least 18 years of age
- High School Diploma or GED required, or experience/ability to provide services according to a plan of service
- Be able to pass a background check with the department of health and welfare
- Have a working cell phone
- Have basic Technology/computer skills
- Valid Drivers License and Vehicle Insurance maintained in personal file
- Must have reliable transportation and transport clients in own vehicle as required per client specifications
- Maintain a valid CPR, First Aid, & AED certificate for adults
- Must obtain a Assistance with Medication Certificate
- Previous experience working with the mentally and physically disabled population
